Before we (or rather, the experienced dog people here, haha...) can really tell what the problem is, could you give us a schedule of a typical day in your house? Wake-up time, walk times, training times, feeding times...just what the day's like.

If she's not going completely outside, she's not completely potty trained. Look at the potty training thread, keep her in sight at ALL times, and follow the advice in the pinned potty training thread. She might have been potty trained at another house, but sometimes dogs feel there are different set...

Well I don't know about Congress, but today Opal slipped out the door while people were installing our new TV stuff. She went outside, ran around a tree, then came back in and looked up to where we keep the treats. :| We didn't even have to call her back! Haha. :)

Often dog groomers are better than vets - do you know of anyone in that area who can be trusted? On sensitive dogs I just clip one nail a day. Try that? Clip, reward, end of, for that day. Onyx seems to get a new health problem every winter that the vets never know what to do with...might be similar. At any rate, it goes away come spring. Then, of course, it's too hot for him to do much during the summer. :|

I don't know if the intertube-like collars could stop a dog licking their front paws, but I have no experience with them. Have you tried putting an old sock on her foot, or another covering like that? I know most dogs don't like stuff on their paws, but it might be preferable to the e-collar.
